WEE Series Questionnaire

This questionnaire should only take 5 minutes to complete and we would appreciate your comments . It is intended to use the comments supplied to improve the WEE Series in 2006. All questionnaires completed before midnight on Sunday 4th September will be entered into a free draw to win a complementary membership of LVO from now until the end of 2006! The draw will take place at the LVO committee meeting on Monday 5th September.

Please feel free to fill out the form even if you have not attended any of the events in 2005 (however, the free draw is only applicable to people who have participated in at least one event during WEE 2005).

Each person in a household is free to fill out the form individually.
